This Service Agreement ("Agreement") is made and entered into effective as of [Date], by and between:

**1. SERVICES**
The Service Provider agrees to provide the following services to the Client (the "Services"):

**2. TERM**
This Agreement shall commence on [Start Date] and shall continue until [End Date] or until the Services are completed, unless terminated earlier in accordance with the provisions of this Agreement.
**3. COMPENSATION**
In consideration for the Services provided by the Service Provider, the Client agrees to pay the Service Provider the following:
